Darst Dermatology is conveniently located at the intersection of I-485 and Providence Road (NC-16) on the south side of Charlotte, NC.
A visual reference would be the names of the other practices we share our building with: Ghorshi Family Dentistry, Queen City Eye Center, and State Farm. Our building is called Belle Rea Building. We are adjacent to Bank of America (share a parking lot), our front door faces Providence Road and there is a medical building behind us. On the same side of the road is Rea Village that includes such businesses as: Ilios Noche, the US Post Office, Post Net, Carvel Ice Cream, and Hickory Tavern.

We are in the medical complex behind Olive Garden that is across the street from the new Target. We are in the same building as Southern Piedmont Primary Care and the Urgent Care.
Hours
Dr. Darst sees patients from 8am to 5 pm, Monday through Friday.
We are closed for lunch from noon to 1 pm.
(There is an answering machine if you need to leave a message over the lunch hour)
Appointments
We are accepting New Patients! We treat the entire family including infants.
New patient appointments are available within a few days, especially for urgent conditions.

Darst Dermatology offers its patients a personalized
Patient Portal through which they can communicate with the practice, fill out pre registration forms, request and reschedule appointments, receive biopsy results, and more!

Patient Forms
To help decrease your waiting time in our office, please download complete and return these forms prior to your visit. For your convenience, these forms have been formatted so that you may fill them out using your computer. You may leave the signature line blank and sign it at your appointment.
If you choose, you may email the completed forms to us at

Alternatively, you may fax the forms to us at:
704-541-6558. Or, if you are in the area, please feel free to drop the forms by the office.
6 Individual Documents
If you choose not to fill out the forms until you arrive at the office, please arrive at least 30 minutes prior to your scheduled appointment time to complete these tasks.
If your forms are not filled out by your scheduled appointment time, you may be asked to reschedule.
Insurance & Billing
Darst Dermatology participates with most insurance plans. See below for a partial list of insurance plans. If you do not see yours listed, we may still be a provider- please call the office to check at 704-321-3376.
We request that you have a current insurance card with you at each visit, so that we may keep your information up to date and file claims efficiently.
For insured patients, we request the patient’s copay at the time of service. If needed, we will work with you regarding a payment plan on an individual basis.
Our business office is available for any other questions you may have about your insurance and payment plans.

Referrals
Dr. Darst does not require you to have a referral from another doctor to see him.
You may schedule your own appointment directly with our office.
Some insurance companies, however, require a referral for the visit to be covered.
Please contact your insurance company to obtain that referral if it is needed.
As a patient, it is your responsibility to obtain any needed referral authorization prior to your office visit.
